Planning on Diving? You must fill out a Diver Medical Participant Questionnaire for your dive company. If you answer YES to questions 3, 5 or 10 OR to any of the questions on page 2, a medical clearance is required before you can dive.

Step 2: A licensed medical provider reviews your health history and assesses dive fitness.
Step 3: Clearance is issued promptly and coordinated with your dive shop-if eligible.

About Dr. Laura Burns DFM FNP-BC PhD

I have been a certified scuba diver since 2006 and have traveled the world exploring incredible dive destinations. Diving is not only a passion of mine, but also a lifestyle I share with my whole family. My husband and all four of my children are dive certified, beginning at the age of 10.

As a licensed medical provider in Hawaii, I am very familiar with the medical and physical demands of diving and the fitness required to dive safely. I combine real world dive experience with medical expertise to provide efficient, affordable dive medical clearances.
